Grant Guidelines
Mission Statement
 Wise Up!, an initiative of the Al and Sharyne Wallace Family Foundation, has been organized for the purpose of educating the public on the various issues surrounding concussions, as well as raising research dollars for the advancement in concussion research. Awareness with issues over concussions is at an all time high, but unfortunately the research is lacking. Our goal is to help further research and continue to educate the public as new findings come available

When are applications accepted?

Applications are accepted on an ongoing basis.

How are applications reviewed?

Grant applications are reviewed by the Wise Up Board of Directors on a regular basis for consistency with our mission, geographic focus, and types of support we have elected to provide in a given year.

General Guidelines:

Organizations must have tax-exempt status under IRS section 501(c)(3)

Grant Guidelines:

Within our funding guidelines, we consider grant requests without regard to race, gender, disability, religion, ethnicity, age, or sexual orientation. Wise Up! will carefully consider every application submitted; however, it is impossible to fund every application received.

Geographic Focus:

Types of Support for Fiscal Year 2014:Primary focus is Southeastern United States (AL, AR, GA, FL, LA, MS, SC, TN)

Education: Concussion education programs, especially programs that can easily be duplicated in other areas and reach a wide variety of groups.

Research: Research being conducted to study traumatic brain injury and the prevention of and/or lessening the effects of concussions.

Application Checklist:

  1. A Cover Letter, on your letterhead, signed by your Chief Executive Officer or Board Chair.
  2. Grant Request Summary to include primary grant applicant contact name, title, organization's name, mailing address, telephone number, email address, website address, project title, program summary, amount being requested, and an overview of alignment with the mission of Wise Up! (Limited 2 pages)
  3. A Proposal (no more than 10 single spaced pages in 11 point font or higher) that includes:
  • Agency Background (mission, major activities, and credentials for carrying out project)
  • Project Description: Brief statement of problem to be addressed
  • Goals and Objectives
  • Who is Served
  • Project Activities
  • Expected Outcomes
  • Staff (overview of positions and credentials)
  • Plan for Measuring Project Results (*For renewal requests - an update on progress made through prior grant)
  • Project Budget (include requested amount, total project budget, other funders and amounts)
  • Information about your Organization: (Agency's Operating Budget, A list of Board of Directors, Organization's 501(c)(3) Letter, Latest Audited Financial Statement, Form 990, Latest Annual Report(if available).
